Tooth extraction

When the tooth is too damaged or it cannot be sufficiently supported because of the state of the surrounding gum or jawbone, its removal might be necessary. First of all, our dentists make sure that the decayed or broken tooth cannot be fixed in any other way, like a crown, a filling, an inlay/onlay or root canal, then they decide to apply the extraction. Please find below some examples of when it is inevitable:

  • very extended decay when the restoration is impossible
  • moving tooth because of caries
  • accidently broken tooth because of trauma
  • painful wisdom tooth
  • delayed loss of primary tooth that hinders eruption of permanent tooth
  • in case of some surgical interventions
  • in case of unnecessary tooth eruption

In most of the cases it is a simple process realized under local anaesthesia when one part of the tooth can be reached with special dental tools over the gumline.

Surgical tooth extraction

However, in other cases it is necessary to apply some more complex techniques than a simple extraction. In case of an extremely extended decay or when the tooth is having curved or diverging roots, surgical tooth extraction might be necessary. It is also advised in cases when largest part of the tooth is situated under the gumline or not even erupted. The oral surgeon makes a small incision to remove the broken tooth then.

Also wisdom teeth have to be treated in a surgical way when they are impacted and endanger the neighboring teeth and/or become painful

Bone grafting

Before starting a dental implantation procedure, the dentist is evaluating the state of the bone. If its density and/or quality are not enough to support the artificial root, bone graft might be necessary to help the jaw to anchor the implant.

Bone loss can be a result of a long time missing tooth in the jawbone or chronic periodontitis, for example.

Dental implantation soon after tooth extraction or loss can be a good solution to avoid natural bone loss in the affected area. If it hasn’t been happening, we can expect the necessity of augmentation of the bone during a procedure also called bone grafting. During the process we elevate the bone by adding graft material before the tooth replacement. This material can be artificial or natural.

Artificial bone graft material

The name of the artificial graft material is alloplast. It is a synthetic, biocompatible material that mostly contains Hydroxyapatite, an essential ingredient of the tooth.

Natural bone graft material

It contains harvested bone that’s been processed to decontaminate and supplemented with necessary minerals. Along with the substances added, this material provides a framework for the patient’s surrounding bone tissues to grow inside the hole and fill it properly.

Bone transplantation

This third type of material is different as it is a more complex intervention. An autograft is realized from the own bone of the patient (hip, shin, scull or jaw) that we use to augment the bone quantity of the patient at the damaged area. This piece of bone is attached by osteosynthesis screws.

Most of the time we also use resorbable membranes to cover the material for a better regeneration of the bone and the tissue.

Regeneration takes around 6-8 months following the operation.

Am I Suitable candidate for bone grafting?

General health and oral hygiene are highly important to get a successful operation done. In case of uncontrolled diabetes, for example, it is not possible to have this kind of a severe surgical intervention.

Is bone graft material safe and reliable?

Procedure of bone grafting

Steps of bone grafting (bone loss - added bone grafting material - dental implant in the treated jawbone)
  1. It can take 30 minutes but even 3 hours, depending on the location and the nature of the bone grafting procedure.
  2. The whole operation is happening under neuroleptic analgesia and local anaesthesia to make the intervention completely pain free.
  3. After the bone graft, we need to wait 6 to 8 months to get the dental implantation process started, though in some cases, it is possible to start it right after bone augmentation.

Sinus Lift

In case the bone volume or quality is not enough in the upper jaw bone, we talk about sinus lifting. It is made with the same method as simple bone grafting but only in the area of molars and premolars, over the maxillary. The simple bone grafting procedure is being completed by the lifting of the sinus membrane while we get some more space for the grafting material to provide more density. This technique exists for over 20 years and has a proven record of being a successful solution.

Dental implantation procedure

Guidelines after oral surgery

We need to handle carefully the post-operational area after an oral surgery intervention. We would kindly recommend you to take the following advices to serious after the operation:

  • avoid eating and rinsing for a couple of hours
  • try to chew on the other side of the mouth for a few days following the oral surgery intervention
  • do not remove the blood clot to avoid infection and help healing
  • avoid drinking alcohol, coffee, dairies and eggs
  • no sports, sauna or thermal water for a few days
  • avoid eating under local anaesthesia (lasting 2-5h after the end of the treatment)
  • clean carefully the affected area, we recommend you to use soft brushes for that part of your mouth
  • do not use mouth water until the healing process is over
  • no hard nose blowing is advised because of eventual maxillary problems
  • try sleeping on the opposite side of the affected area or on your back
  • use ice jelly on the face and moisturizing cream underneath
  • call immediately your oral surgeon in any case of serious complication, like excessive pain, bleeding or fever

What are the risks of oral surgery?

Medical complications may arise in any kind of surgical operation. Medicover Fogászat is trying to make sure, these risks are minimized by working only with a highly qualified and experienced medical staff as well as the highest quality materials available on the market. If you feel like having excessive, unstoppable pain that cannot be resolved by the use of painkillers, if your face is too swollen for longer than a week, you should consider the possibility of having some complications.

Complication with the sinus

One problem that may arise is the opening of the sinus after a tooth extraction in the upper jaw. It needs to be treated immediately, even though it’s not as dangerous as it seems at first sight.

The sinus has to be closed with some stitches and handled carefully until final healing.

Risks of local anaesthesia

  • Allergic reactions: very rare
  • Local haematoma: disappears in 1-2 weeks
  • Damage of the nerve: rare, it might happen in case of a mandibular block. This can cause sensibility disturbance in the cheek for a few months. The nerve is able to regenerate by itself in months.
  • Restricted mouth opening: lasts for 1-2 days
  • Temporary facial palsy (affects the mimic muscles) which disappears in 2-3 hours
  • Temporary dullness, fading

Risks of dental implantation

  • avoid driving a car shortly after the intervention
  • no sport or sauna to decrease risks of bleeding
  • use of ice jelly is recommended, application of moisturizing cream under the ice is advised
  • avoid eating under local anaesthesia for 2-5 hours, drinking is allowed
  • try eating on the other side, mainly soft food ; consuming alcohol, coffee, dairies and smoking are not recommended
  • do not suck on the wound, nor rinsing is advised as they are increasing risks of infection
  • keeping proper oral hygiene during the healing process is primordial from the point of view of the successful integration of the dental implant, the use of soft brush is recommended, mouth water is not advised in the first couple of days following the operation
